3. Pull up the form from the link below then click your mouse in the "Name" field on the form.
   
   
4. Fill in the information, using the "Tab" key to go from field to field.
   
   
5. Print out your completed form and sign it. (This form cannot be emailed -- we need your signature.)
   
   
6. Mail or fax the form to us with your deposit check or credit card information.

TRAVEL INSURANCE
For travel and trip cancellation insurance, we recommend Travel Insured International.

You can purchase a comprehensive protection plan, or a post-departure protection plan, in one of three ways.
